all 4 types of indexes now have different icons for enabled, disabled, filtering, filting with errors

triggers have different icons for enabled and disabled.
the detail view is also showing the correct icons so I had to modify all the extractions for triggers & indexes. Constraints extraction was already retrieving the needed information so no regression testing needed.
Verified in ADS v15.0.0-rc-34. See attached screenshots. Details view is also fixed and status icons are displayed correctly
