This happens if you try to open a Query Window which fails as in the indicated screenshot, and then the user chooses the application menu item Query->[Change Server].

This is now resolved in ADS v16.0.5-14
I made an additional fix to get the text from the original Query Window and display it in the new Query Window after Change Server is successful.
