Primary Key Options for Azure ER Modeler should match the Create Table GUI

For the Primary Key Constraint details for a Windows Azure ER Model, it should match our Create Table Visual Editors

Currently there are a few options in the Primary Key which are not scripted. We need to remove those from the ER Modeler
